*A $50 deposit is required to secure your appointment, and will go towards the total price of service. Deposit is refundable if client is not a good candidate for PMU. Appointments will not be confirmed until the deposit invoice has been paid.
*All clients must fill out our Google Medical Form before an appointment to ensure the procedure can be done safely. A form will be emailed, or sent via text message to be completed.
*After an appointment is scheduled, you will receive a Pre-Care instruction PDF via email or text message.
*Client must be 18 years of age, or older. Certain situations that are medically related or hair loss related may be considered. It is critical to abide with the law that minors MUST be accompanied by a parent or legal guardian.
*An informed consent form must be filled out and signed during appointment right before the procedure.
*No-Show Policy: If a client does not show up to their appointment without rescheduling/ notifying me any time before said appointment, they must pay a $100 fee.
*Consultations are free, and highly recommended if you want to learn more or unsure about anything PMU related; However, they are not required.
